simplest and most efficient is probably to go to small
claim court to recover the amount from
your insurer.

The
final responsibility is unfortunately yours.
Can you recover from the doctor?
If you have signed a billing policy with the practice there is
little chance you may recover. Doctors and practices need to be paid to
continue treating patients like yourself. This is why they can’t take full
responsibility for your insurance defaults. More and more practices have had to
close because of managed care defaults. You may also find that less and less
doctors are available on your insurance plans or that fewer and fewer
practices are willing to accept insurance plans as payment.
